De olika tekni- kerna var kryssincision i fascian, cir- kelincision i fascian och nätförstärk- ning i bukväggen. Delarbete 5 är en randomiserad, kontrollerad studie. Totalt 209 patienter randomiserades i de tre armarna av studien: n=74 krys- sincision, n=72 cirkelincision, och n=63 i profylaktiskt nät. Parastomalt bråck bedömdes med CT buk ett år postoperativt men också genom klinisk bedömning av kirurg samt stomisjuksköterska. Vi fann inga signifikanta skillnader i förekomsten av parastomalt bråck ett år postope- rativt inom de tre olika kirurgiska tekniker som användes för stomi- konstruktion. Konklusion Identifiering och hantering av pre- operativa riskfaktorer, stomitypval samt medvetenhet under konstruk- tionen av en kolostomi kan leda till en minskning av stomikom- plikationer och förhoppningsvist förbättra stomifunktionen samt patienternas livskvalitet. Det finns inget samband mellan någon av de tre jämförda kirurgiska teknikerna för stomikonstruktion och förekom- sten av parastomalt bråck. I nulä- get är användning av profylaktiskt nät under kolostomikonstruktion rekommenderat av European Hernia Society Guidelines 20 för att minska incidensen av parastomalt bråck. Graden av evidensen är fortfarande låg 21 och därför måste denna rekom- mendation tolkas med försiktighet. Resultat av såväl vår studie som en annan svensk publicerad randomi- serade studie 18 har inte visat någon fördel i användning av profylaktiskt nät kontra icke nät, ett faktum som gör att den frågeställningen fortsät- ter kvarstå.
